Uninstall xml not being generated

In a random fashion - I find that the <package>irunin.xml file is not generated during the installation.

This is not on a per-kit or per-machine basis (i.e. a specific build of my application will generate the uninstall file on one machine but not another. The machine that does not generate the uninstall file one time will generate an uninstall file the next time).

Is there some way to identify why the uninstall file does not get generated?
